Why changes queue for weeks

An MOC form is a set of claims about impact. Someone has to establish them, and today that means a manual search across drawings, procedures, and records that no one system holds.

  • Discovery is a plant-wide question. A one-valve change can touch connected systems on other sheets, a relief path sized for the old configuration, an interlock description, three procedures, and an operator training module. Each lives in a different place, and each has to be found by hand.
  • The form only shows what the search found. The impact that was not discovered does not appear on the analysis, does not get a reviewer, and surfaces later as a startup problem or worse. The quality of the MOC is set before anyone signs it.
  • “Temporary” outlives its authorization. Temporary changes carry expiry dates that pass quietly. Extending is easier than removing, and the plant accumulates configurations nobody decided to keep.

Process safety management under OSHA 29 CFR 1910.119 expects a change's impacts to be understood before the change is made. The expectation is right. The manual search behind it is why changes wait weeks in the queue.
